  • Middle School Athletics Requirements and Registration

    Union County Public Schools is pleased to partner with Family ID for online registration! These forms must be completed prior to any involvement in pre-season skill development sessions or tryouts.

    How to register to participate in middle school athletics in UCPS:

    NEW STUDENT ATHLETES:

    Step 1: Create a FamilyID Registration. Be sure to select all sports your child wishes to play this year. Please only create one registration, then edit if corrections are needed.

    Step 2: Download and complete the NCHSAA Preparticipation Form (physical).

    Step 3: Upload the NCHSAA Preparticipation Form on your FamilyID registration.

    Step 4: Submit your completed FamilyID Registration.

    You will receive an email notifying you once your registration has been approved. Please allow time for staff to process the registration for approval.

    RETURNING STUDENT ATHLETES THAT HAVE USED FAMILY ID PREVIOUSLY: 

    Step 1: Do not create a new FamilyID account! Log into your FamilyID account and submit a registration for the current school year. New sports years begin in July.

    *If you do not remember your login, please contact your schools Athletic Director.

    Step 2: If your physical is still valid from the previous year and on file with the school, you do not need to upload a new one. Please be aware of when your physical is set to expire. It must be valid for the entire season.

    You will receive an email notifying you once your registration has been approved. Please allow time for staff to process the registration for approval.

    By completing the FamilyID registration and the UCPS Athletic Eligibility packet, my child and I have read and understand the athletic eligibility requirements to participate. I understand, any forgery or failure to properly sign the information contained in the eligibility packet (including Family ID online registration) may result in fines and penalties to my child’s school and/or the ineligibility of my student-athlete for 365 days.

    Don’t Get Caught Without a Valid Physical!

    The NCHSAA states that in order to be eligible for practice or participation in high school athletic activities a player must receive a medical examination once every 395 days by a duly licensed physician, nurse practitioner, or physician’s assistant. If the athlete’s physical expires they become ineligible and can not practice or play until a current physical is on file with the school. This includes in season and out of season workouts.

    Even if new physical is not required, a new Pre-Participation form is required for this school year.  It is the first page of the Physical that is filled out by the parent and athlete.
